How the Plan Works

Under the CSR Open Access HMO plan, you pay a flat fee, or copay, for most covered services (such as $15 for a doctor's office visit). There are no deductibles, coinsurance or claims to file. When you need care, you visit any network health care professional or facility and pay the appropriate copay. You are not required to select a PCP and no referrals are required for specialists.

It's important to remember that the plan pays benefits only when you receive care from a network provider. If you go outside the network, you pay the entire cost of your care out of your own pocket.
