Claim Fiduciary

Aetna is the fiduciary of the CSC Signature PPO, Be Well Premier and Be Well CDHP, CSR Managed Choice POS, CSR Choice POSII Plan, CSR Open Access HMO Plan, CSC and CSR flexible spending accounts (FSAs) for purpose of reviewing denied claims under these plans. In exercising such fiduciary responsibility, Aetna will have discretionary authority to determine entitlement to Plan Benefits as determined by the Plan Documents for each claim received and to construe the terms of the Plan. Aetna's decision of any claim is final and Aetna has no other ERISA Fiduciary responsibility under the Plan, except as required under ERISA.
